Care Coordinator – Domiciliary Care and Supported Living

Hourly Rate:  13.5 – 18.50

Location – Essex and Environments (Flexibility is paramount to us)

Successful candidate will work closely with the Care or Service Manager to meet and plan outcomes for our service users in the day-to-day line management and supervision

The Care Coordinator is responsible for all tasks related to helping with new client starts including matching and scheduling carers to clients. Managing Ad hoc rota

As the Care Coordinator, you will support forward planning of scheduling and ensure it is consistently achieved in order to provide continuity of care. You will ensure all clients are safeguarded against the risk of abuse and respond appropriately to any allegations.

  • You will liaise with external stakeholders and agencies such as next of kin, GPs, Nurses and Physiotherapists – prioritising contact where the wellbeing of the client is being considered.
  • You will document any care worker absence using our electronic rostering system, and be part of the on-call rota system – covering weekends and evenings where required.
  • You will be required to cover care calls where necessary across clients’ homes and venues
  • You ensure continuity of care provision through effective contingency plana and be an integral part of the Branch office team.
  • Staff Induction, Training and development and Compliance with regulations and regulator including Social Services, CQC among others

 

About You

Our ideal Care Coordinator candidate will have;

  1. High attention to detail gained in a previous administrative or customer service orientated role;
  2. Good level of IT knowledge, in particular MS Office skills
  3. Previous experience working within social care
  4. Experience of co-ordinating rotas or schedules
  5. Can do and will do spirit
  6. Have foresight and be supportive, considerate, flexible and compassionate
  7. Experience of Staff plan is essential
  8. Ability to work under pressure with or without supervision & according to business demand.
  9. Be there for emergencies to accept packages and assess immediately, according to business demand & business development.
